I was also not clear on what that meant.  In the facility I work at LPN's do initiate some of the care plans.  Care plans are interdisciplinary so why wouldn't an LPN be able to develop a care plan?  LPN's are able to complete the MDS as long as an RN signs off at R2b. They can also do raps.  An RN  needs to sign off at VB1 and date it at VB2.  The MDS manual states that anyone can sign at VB3.  It states it can be any person(s) who facilitates the care planning decision making.  (page 4-17).  So I am also confused about what was said about LPN's and care planning.

I am not clear on a LPN not being able to develop a care plan-what exactly does that mean?  Does that mean the RN must write all the care plans and the LPN can then update them as needed?  Please elaborate on this since the LPN's I have ever worked always did their own care plans and now that scares me a little since I was unaware of this.  Thanks
