MY PREVIOUS FACILITY'S CORP POLICY DICTATED THAT WE DO THIS. I USED "POTENTIAL FOR POLYPHARMACY" AS THE PROBLEM..... GOAL OF "NO ILL EFFECTS FROM POLYPHARMACY (or something like it)"...... INTERVENTIONS OF ROUTINE REVIEW OF MEDS BY PHARMACY, MONITOR FOR INTERACTIONS, ETC... I CURRENTLY DON'T CAREPLAN IT AT CURRENT FACILITY.
