This began as a patient safety initiative with JCAHO. In my former life of
acute care we tackled this only with verbal orders- requiring a readback for
verification. Had it in our policy that we "readback" verbals and did not
require two signatures which seems labor intensive.

        Are there any facilities out there that require two nurse signatures
when transcribing an MD order (essentially a double-check)? We do require
this and it has not cut down on our transcription errors. I think the
nurse's are just signing without actually checking to see if it was
transcribed correctly (obviously). Before I start a campaign to drop this -
I wanted to get a feel from you all on what you are doing? Thanks in
