First off, is every diagnosis currently being treated?  Doubt it.  Also, this would mean that her nurses and aides would have to be documenting according to the care plans and actually performing the plan of care as written.  This is impossible and a real huge flag for surveyors.  The facility must have been cited in the past r/t care plans and she is trying to overcompensate.  I will look through what I have to see what I can send you that would be Official, so to speak.    Sherri
