The National Institute of Health's instructor J. Breedlove RN suggests in her programs that facilities write" MD Orders/ Nursing Plan of Care", on all MD order sheets. This allows for the MAR & TAR & order sheets to be a good & updated record of the plan of care, ( so that the multidis care plan is really multidisciplinary). We have not gone to this yet but hope to do so.

This is impossible and a real huge flag for surveyors. The facility must have been cited in the past r/t care plans and she is trying to overcompensate.
