My former facility was just surveyed by the Lehigh Valley office. They were cited for lab work being missed (D), pharmacist recommendation not being acted upon nor note from MD (D) and one other D level that escapes me right now, I think it had to do with a discharge record. They were looking closely at falls, siderails, and care plans. they were very nice. Good luck
