When Ifirst started as an MDS coordinatorm they were doing the same thing.  my thing is you have to write on the careplan with balck ink why make more work for yourself by having to also pick up a yellow highlighter.  So we started just drawing a line through anything.  The surveyors had told us that yellow highlighter does not copy so it looks like we didn't do anything.  I agree the yellow highlighter makes it look prettier but I go for short and simple.  WE have enough work to do as it is why make our jobs more difficult.
