I also have a simalar situation, but I was told that if the therapy is discontinued during the 30 day, all you have to do is an OMRA.  According to the response here. I do the 30 day and then 8-10 days later do the OMRA.  I am new at skilled and obviously still confused.

Yes.  And if he continues to receive any other skiller, an OMRA is due 8-10 days after the last day of therapy in order to change the RUG level for billing.  Any chance of therapy reducing him to 3days a week and letting restorative pick up the balance for a week?  Will get you a rehab low for longer period of time.  Just a thought.
