I would submit them now, though late. Place the correct dates in the date of discharge and reentry and sign for today. It will show as late, but better than missed.

Hi all,
If a discharge and re-entry tracking form were not done and this is discovered several months later on a resident that was Medicare A stay should they be done now?
Resident was admitted 12/12/03, 5-day MC/admission MDS completed 12/17/03, was discharged to hospital 12/19/03 and readmitted 12/21/03, then died 12/22/03. A discharge tracker was sent 12/22/03 but her brief admission to the hospital tracking forms were missed. Anyone know what to do at this point?
