At my facility we get an order for restorative nursing which includes how 
many times a day, how many times a week, what extremities and what type of 
restorive nursing we are doing such as PROM to upper extremities once a day, 
five days a week.  We get orders for every service we provide so that 
everything is uniform and tidy.  That way services are not lost in the 
shuffle.

we do not get a physician order, but usually we get reccomendations from 
therapy, prior to starting the resident on nursing restortative.
