Beyond any exceptions they have to be picked up within 30 days. However, there are 
exceptions and they are explicit, see "Medicare Benefit Policy Manual", Chapter 8, 
starting at 20.2 "Thirty-Day Transfer".

I have a resident who just returned from 3 day qualifying hospital stay.  She has 
passed the 30 days where I could have picked her up again had she had a decline in 
condition and needed further skilling.  She has not had a 60 day period of wellness.  
Can I skill her and use the remaining 38 days that she had under her initial 100 days. 
 Part of my MDS team is saying yes, once she starts her initial 100 days, if she has 
another 3 day hospital stay then she can use up whatever she didn't use before she was 
d/c'd to LTC.  The other part is saying that it's a shame but if she has passed the 30 
days but has not a 60 day period of wellness then she can't continue to use up the 
remainder of the 100 days she didn't use before.

Also, when you reply  please tell me where I can find this in writing.  One half of 
MDS team is going to want me to "prove it" Thanks.
