Toileting plan

Does anyone have an example of a toileting plan/program that is appropriate to be coded on the MDS. All we have written in our facility is plans such as-toilet before and after meals and activities at bedtime and upon rising in am. We have judged that this is not what the MDS means by a toileting program specific to the individual-are we right? Any input is appreciated. Becky
