An individualized toileting program requires an assessment of the resident's abilities, type of incontinence, and particular voiding pattern.  Once you know WHY they are incontinent,and what their mobility and cognition are, then you can develop a plan to keep them dry.  This may involve habit training, bladder retraining, or simply following their voiding pattern to "catch" them before they need to void.  None of this can be determined unless you do an assessment though.

Does anyone have an example of a toileting plan/program that is appropriate to be coded on the MDS. All we have written in our facility is plans such as-toilet before and after meals and activities at bedtime and upon rising in am. We have judged that this is not what the MDS means by a toileting program specific to the individual-are we right? Any input is appreciated. Becky
