This resident is a paraplegic with swallowing problems and weight loss
issues. If his po intake is not greater than 50 % of each meal, he gets a bolus.
Hasn't had a bolus in past year and weight has been stable. When he was
admitted, he had a huge pressure ulcer that caused him to lose his entire right
leg and end up with a colostomy. All healed now, but remains at risk for skin
breakdown.
