Restorative flow sheets

Hey Gang,   Does anyone out there have a decent restorative flow sheet that you could email or fax me?  I am (once again) serving as interim DON at this wonderful facility, and our restorative aide came to me today to ask, "Is there a sheet somewhere to document when I ambulate someone?"
