I am not sure what you are asking. Using the grace days as a matter of routine on a 5 day assessment could set off flags among surveyors and FIs. That does not mean it always indicates a problem, but it could draw some attention.
 
For most facilities, simply using grace days will not result in an ultra high score. If using the grace days does result in a higher score than not using grace days, and really gives a better indication of the care being delivered to the resident, then perhaps you are justified in doing so.  It would be perfectly acceptable to use grace days 100% of the time IF you have a good reason for doing so each time. I am sure that facilities that specialize in high levels of rehab use the grace days more than a facility that does not.
 
Project minutes are there primariliy for situations where rehab might not be started right away. Some residents may be to weak to start rehab in the first week. Without projection, they may not even be eligible for skilled care.
