I would do a significant change at this time. The fact that therapy has been D/C'd and the Rt is leaving the rehab unit is an indicator that they have plateaued. At our facilities we use the therapy discontinuation as our guide (Rt is evaluated for sign. change at that time) to the resident plateauing and doing the significant change (unless of course therapy was d/c'd because they did not progress). I am in CT - we have found our surveyors also use this as a guide. Good Luck

Subject: Question re: SCSA

I have a resident that has been on our rehab unit and has been working with therapy.  The resident will be going to our LTC unit and will no longer meet skilled criteria for nursing or therapy.  My question is if the resident has met the goals that therapy has set forth and is now functioning at that level do I need to do a SCSA because he has now plateued at that level?  My raps indicate that we are anticipating improvements toward therapy goals.
