This is the issue... what was this rope secured to? If an accident were to occur, what is the worst thing that can happen to the resident(s) in the van? Read appendix Q on Immediate Jeopardy. While your facility hasn't had an accident to date, there is a possibility. If you have to run a rope through the wheels for an extra source of stability and security, what's wrong with the locks? I can empathize with you in this case. I've heard of EMS/Transport companies cited, fined and sued for faulty locks. Good Luck!
