This is strictly off the top of my head but I think that MDS helps most in resident care by having a team thoroughly assess the resident, his/her care plans and the medical record to find out if anything could be improved.  I don't find RAP's very helpful, sorry.  We usually have care plans complete before RAPS are done.  Sometimes they point out omissions in the plan of care but not usually. I mailed my survey a couple of days ago.
