I see the MDS as as starting point and then the investigation with the raps,just remember Sherlock Holmes and then the plan of care to hopefully maintain or improve a res quality of life. I'm not saying the process is perfect but I think it helps
. We often see things or find things that the floor nurses would not  have found. We have a care plan team that works very well together and have excellent communication with one another We work together  to be creative to solve problems..I believe two heads are better than one is very accurate.
