If advancing diet then probably more than one other change could be happening.  increased independence in eating, cognition, alertness, mobility, mood, medication changes, MD or dentist visits. I would have everyone look again for other improvements that are making this diet advance possible.  Look at the therapy minutes/days for the 5 days.  Try to compare the "therapy" rate vs. the OMRA rate and see what the reimbursement difference (total gain) might be.  $250 x 13 days vs.. 200 x 13 days is a $650 dollar gain.  (13 because 5 days of therapy plus 8 days before OMRA would be done again.) What does it cost for the team in terms of time to do another assessment? . 
The real question is, did the resident really reach a plateau and no longer require therapy services? What if Speech therapy could have stepped in on or before the 10th day after the other therapies stopped, this whole dilemma would have been avoided. Sometimes we are too quick to decide that there are no other gains to be made. I know we have made that mistake here before.

Would you do an OMRA if you had this scenario? On 3/17 an OMRA (replacing a 30 day), was done as PT and OT had dc'ed their service. Then from 3/22 to 3/27 ST picked up the resident to see about advancing this person's diet. There was no sig change. Since the resident's rug rate was already a post OMRA rate, is there really a point in doing another OMRA? Thanks All
