Usually we skill for therapy post cva usually much longer than 2 weeks. Often they need all three, OT, PT, and ST, but sometimes you can't work these residents as hard, and it really takes much longer if they are making progress. If they are receiving any other skilled services, ie. wound, O2 etc we can also keep them skilled for nursing if therapy cuts. I would be looking closely and questioning therapy is there is a pattern of cutting CVAs in any routine way. Does this make sense? I am tired trying to get end of quarter work done for case mix!!!!!

I care for all non-Medicare residents to give some background on why I am asking this question. We have a fairly new person (which I am also) doing Medicare. How long do you all keep Medicare residents who are fresh stroke victims? Our Medicare nurse keeps them for approx. 2 weeks and then discharges them. We have a new Social Worker who has been in LTC for sometime. She is trying to tell the Medicare nurse that you can gradually take residents off rehab then they will be covered  through nursing. I hope this makes sense to all of you. Let me know how long you would keep a fresh stroke resident. Thanks Annette
