Holly McGran Holly, Our podiatry consult forms do not match MDS and you can't read his writing anyway!  Our opthamolgy consult forms do have a section that matches the MDS but is rarely filled out.  So we need to work on those areas.  Dental consults are documented with strictly narrative notes on a Dental consult form.
