Our facility has a documentation notebook for each med cart. The nurse on the med cart documents on the patients listed in the front of the book. ADL tracking forms, I&O, 72 hr charting, Medicare charting and anyother important charting is all listed in that notebook. Hope this helps. I am in search of some good info on Medicare charting guidelines. If anyone has any good forms for Medicare charting please let me know, you can email me privately. Thank you in advance.
