Because of the
nature of short term stay we combine the discharge planning and POC together and
try to have it with in 5 days of admission.

Subject: Medicare POC's
Does anyone have any information on is it necessary to do Plan of Care Meetings for strictly Medicare A patients in a SNF?
By the time we get to doing them it ends up actually being more than a discharge meeting, not a POC meeting.
Where can I find info on this? We are thinking of only doing actual POC meetings on OBRA residents in our SNF.
Is it necessary to do any type of discharge meeting with the families before discharging out of facility on Medicare A?
Would it suffice for Charge Nurse or Social Service to discuss discharge plans over the phone or during a visit?
