As I understand the regs ,t says the Rt and family needs to be involved in
the CP process"to the extent practicable". My personal feeling is that when
we contact family re changes in orders or condition ( which we do very
often) we are involving them.
Regard to discharge plan/instructions. that needs to be done from day of
admission to the actual discharge date.

Subject: Medicare POC's

Does anyone have any information on is it necessary to do Plan of Care
Meetings for strictly Medicare A patients in a SNF?

By the time we get to doing them it ends up actually being more than a
discharge meeting, not a POC meeting.

Where can I find info on this? We are thinking of only doing actual POC
meetings on OBRA residents in our SNF.

Is it necessary to do any type of discharge meeting with the families before
discharging out of facility on Medicare A?

Would it suffice for Charge Nurse or Social Service to discuss discharge
plans over the phone or during a visit?

Thank you for any help with this question in advance.
