As I understand the regs ,t says the Rt and family needs to be involved in the CP process"to the extent practicable". My personal feeling is that when we contact family re changes in orders or condition ( which we do very often) we are involving them. Regard to discharge plan/instructions. that needs to be done from day of admission to the actual discharge date.
