We use an admission care plan that is started on our long - term
admissions on day 1 of admission and added to as needed until the 21
comprehensive care plan is completed. We use the one posted on the AANAC
website (modified to our facility). We also have interim's -we pull for
acute problems i.e. UTIs, URI, pneumoniae, skin tear - they are pretty
much routine interventions with an area to individualize - they are
removed when the acute problem is resolved. We also have a seperated
Care plan for our short term rehabs - for they are reviewed every 2
weeks until discharged or become long-term. Hope this helps.

does everyone use interim care plans? my facility never did before and
now is implementing it. do you use a standard one then at 21 day use
their own complete cp or do you just target a few of their obvious
problems  at first?
