Another suggestion. When you stated "that everything is above board," I am
not certain what you meant. If some of the documentation is not "above
board," you may not directly make changes. However here are some things you
can do: in the situation where a physician has not signed a cert or re-cert.
the administrator or HIM Director contacts the physician, has him/her come
into the building to make a LATE-ENTRY which is documented as such. Or, if a
therapist "boo-booed" there should be a "quality report" from the Department
Head stating that the documentation was reviewed and any questionable
documentation was reviewed with the therapist and then clarified in the
report. This is one reason why ongoing QI of therapy documentation is so
important, don't wait until a crisis before correcting a possible problem
with a therapist's documentation.
