Check the CMS website and search under "medical review"...or, alternatively search your FIs website....AdminisStar has published such a checklist and it is posted both on their website and on the CMS website.

I send all documentation in the clinical record for the time frame in question. This would include, at least, the integrted nursing notes, physician orders, rehab., all pertinent MDSs. I send more only if the FI requests more. If the claim is denied beyond the reconsideration and goes to a fair hearing, you may request of the FI to bring additional information. At that time, if it is pertinent, take additional information such as supportive letter from family, cna flow sheets not on the clinical record, etc. It isn't always necessary to send the entire clinical record. Carefully read what the FI asks for and then send only that information. Also, many FIs have a standard list which they send stating whet they want to review.
