One are does not necessitate a SCSA. With issues like incont, wt loss etc
skin breakdown will occur. so look for indications.

Help. If someone develops a breakdown and has no other changes in any areas 
do you do a sig change? I understand that a sig change is warranted if there
is 
a sig change in two or more areas. I have been doing a quarterly or annual 
early when their healed to get them off the qi reports. Is this what
everyone 
else is doing. thank you.
