Hi Tom.. 
For a new PEG, you would want to monitor the site to insure healing and that
no infection develops.  You will have monitoring and cleansing of the site
on a daily basis.  You will also want to assess level of consciousness, HOB
elevated, bowel tones, respiratory status, c/o pain, abdominal tenderness,
vomiting, I & O, bowel movements, and vital signs.  You will need to
document the type and amount of formula you are giving and how often.  You
will need weight and response to enteral feedings as well as documentation
to reflect any P.O. food/fluids taken by the patient.  If the patient is
receiving speech therapy, document response to therapy.  Determine if your
patient is receiving at least 51% of daily calories or at least 26% of daily
calories and 501 cc or more fluids.  Documentation needs to reflect the
daily skilled interventions as well as the patients' response.
