If the ARD on the 5 day was a data entry error (and
it must have been if the resident was actually discharged on the 9th) then the
ARD can be corrected by modifying the record. That should fix the
problem.

While I was off being a good little RNAC at the
AANAC credentialing program, my fill in coworker made a nice mistake...now I
have to fix it. What do I do: she completed and submitted 5 day MDS
(MPAF) for ARD of 4-11-04 and a d/c tracking form with d/c date of
4-9-04. even though "it didn't make sense" she "wasn't sure what to do"
so just went ahead did it as above....ugh! Do I do a sig correction of
quarterly/5day? Is that even an acceptable modifier? Do I leave it
