Same here. I often have that error & in second place is the social security number. Some people do not realize the importance of exact information. Whenever we change admission clerks, I exaggerate & tell them that their mistake takes me an entire hour to correct.  That sometimes makes them a little more careful.

I have a data entry person and sshe made mistakes on about 15 birthdays.  (Before I was employed there)  I had to go back on EVERY mds that had the wrong bday and fix it.  Just as if I had made an error on coding.  Needless to say---the corrections were MANY MANY
I would give anything to get a new data entry person!!!!!!
 
eg.  Pt admitted   1/23/2002  B day was wrong from the first MDS--so I had to correct every MDS done on that patient!!!!!
