She should be coded as a 5-- Medicare return readmission assessment. You do not do a Reentry Tracking form but you code the reentry date at A4a. Yes you continue the OBRA time line they started on admission, so if you have not completed an Admission Assessment you should combine that with the Medicare Return Readmit Assessment.

I have a res. that was discharged prior to completion. She is now back in the facility and I have been wondering should she be coded as a 1 for a 5 day or as a 05 readmission. She has had one 5 day done prior to going back to the hospital. I have always been under the understanding that when a res. returns and you have not done the admission assessment yet you code them as another 5 day because if you code them as a readmission/05 that continues the time line that they were on when they were originally admitted. Please clarify for me. Thanks |
