why are you doing an obra, just do the dicharge tracking sheet and code it dicharge prior to completing initial
assessment.Upon return start over with your 5-day assessment, if she continues to have this trend,then do your
next 5-day assessment as your initial and care plan  if she's still in your facility,if not the cycle will continue to
repeat itself until you can do an initial assessment.
