You would code arterial ulcers in M1. If they are on the foot, I would also code them in M6c.

Arterial ulcers are not included in M2b. See the "coding" instructions on page 3-162 of the RAI User's Manual. Also, see the 2nd example on page 3-164.

I once had a surveyor to say "why" in the rap summary and not "what" since the mds tells the what. So I look at every trigger and try to identify why.
Why what?
Did they want to know why it was triggered?

Yes, why does this trigger or why is this a particular problem for this resident and frequently you will find yourself interjecting the medical diagnosis. I guess that each surveyor has specific points that they are looking for and what pleases one may not please another. Overall, I try to be very detailed in my rap summary --sometimes hard to do if pressed for time. Mary
