Having audited MDS, we recognized a nurse failed to code a colostomy on an
assessment that was completed 6 months ago. In fact she failed to code on
the one 9 months ago as well. This is Rt has had the colostomy for several
yrs so care planning is in place. Do we modify or not. ?
