Section T and the MPAF

My state allows the MPAF assessment on Medicare PPS assessments other than the comprehensive assessment (usually my 5 day). One problem I have discovered is section T. The software I use does not include section T2 on the MPAF.  I have also noticed on all MPAF forms I have encountered section T2 is also missing. According to the RIA Manual it is required on all Medicare PPS Assessments and I cannot find any documentation otherwise. Can anyone help me out?
