Certain Customized Prosthetic Devices are excluded from consolidated
billing, we would have to know exactly which prosthetic you are referring
too?  I have found that most artificial limbs are excluded since they are a
high ticket item.  However, you want to verify that the item in question is
on the list of exclusions.
