Medicare + choice cases are discussed in weekly mtgs with the managed care
representative. This minimizes suprises for the staff and family and helps
enormously with discharge planning. You can find a discussion of the new SNF
ABN on the AANAC website which was posted towards the end of February.

is not for medicare A, but a managed care! Thank you!
But how can you possibly give 48 hrs notice when we call on the day recert is
needed and they tell us, nope , no more coverage?? And I would like
clarification about any changes in medicare A letters as well.

Medicare+Choice as we have never dealt with it here. I think it is
managed care and since a majority of the local physicians here don�t
participate in these types of plans we don�t see it. I was also under the
impression the new ABN letters have to be in use by 10/04 for regular Med
A. Am I wrong? I think I could use some clarification.

is posted, unless I am totally missing something! This is what I
saw:

That IS what it means, isn't
it? And the forms that are there. We're supposed to use
them now, instead of what we were using up until now? To inform
family/residents that their medicare coverage is
ending?

Therapy and SW
work closely together and the resident/responsible parties are
instructed immediately upon any refusal of the repercussions if they
refuse x 3 and this is documented in progress notes. >From there we
follow the guidelines.

I have a question for this
senerio. Our therapists will try for 3 times, then if resident
refused therapy 3 times, they will d/c therapy. If therapy is the
only skill, what do you do when you need to give the family the 48 hours
notice of medicare being stopped? They won't get therapy for those
48 hours. What should be done here? Since now we need to give 48
hours notice and not 24???

hello to all,
i have had a few burning questions. the more i read the worse it
gets LOL here's another ----- OK if a resident
skilled part A for rehab refuses to participate (able to
physically) just refuses either unmotivated or unwilling. i was told
to continue to offer for 3 days before i cut skilled service. on 3rd
day if refused again that would be my last covered day. do you
agree?
