QI hi/lo risk

hello,

               Can anyone tell me if hi & lo risk is only calculated on the residents that already have developed an ulcer? This stratification only shows up when resident has triggered in pressure area. QI does not reveal who's at high risk or not without this.
the only way to know is to determine if the resident meets the criteria yourself,
eg...malnutrition, ext A to transfer ect... am I way off base here, need to know what you think.
