A physician's order is usually NOT required for Restorative nursing.  However, if the restorative nursing is being given for a Medicare A resident, a physician's order is required before it can be used as a RUG item for Medicare. So, a dr's order doesn't hurt, but is not required.  It is usually agreed that a dr's order for restorative cannot be counted as a change of orders for the MDS in section P.
