Psychotropic med review is also part of my duties, as are physical
restraints, falls, etc. We are required to review and reduce or dc q 3
months or q 6 months, depending on what pharmacy consultant is on board at
the time. I just go ahead and review q 3 months to stay ahead of them. I
send out a notice to the docs usually a month ahead of time so they can let
me know if there are going to be changes made. I let them know how the
behaviors are with the use of the meds, and if they have stabilized the
resident we consider reducing the meds. If not, they have to justify to me
in writing why the current meds and dosages need to stay as they are. The
progress note is then made a part of the record in the chart for the
surveyors to see when they go through the charts. I made a check off list
for them and am getting a lot more cooperation now that they don't have to
write a lot.
