Log reduction is just a mathematical term. It means 10 times greater or ten
times less.

So, we use the term log reduction most of the time when we are talking about
CML. If you have a 1 log reduction with your CML it means that there are 10
times less CML cells in your system since the last time you measured it. If
it is a 2 log reduction, it means that there are 100 times less CML cells.
And a 3 log reduction means 1,000 times less CML cells.

In order for this to be meaningful you need to know how many CML cells you
started with at diagnosis time. Since very few of us had a PCR test done at
diagnosis time, each lab makes a guess at what the CML level was at
diagnosis time for CML patients at that lab. You don't have to know what
that number is because all the labs report your results in log reduction.

If you look at my results you will see that I am at a 3.8 log reduction.
This means that the CML level in my system is 1,800 times less than it was
at diagnosis time. A very good place to be.

Many times you will read that someone is at PCRU. This means that the lab
performing the PCR test could not find any CML cells in your system. Well,
it turns out that some labs do very sensitive PCRs and others do less
sensitive PCRs. So, if you took two samples of your blood to two different
labs one result could come back as a 3.8 log reduction and the other could
come back as PCRU or undetectable. All this means is that the second lab had
a less sensitive test than the first one.

In order to be able to see how you are doing, you need to be tested at the
same lab under the same conditions in order to compare results.

I realize that this was a long winded answer and I hope it helped to unravel
the mystery of log reduction.

A couple of numbers to be aware of. If your resut is a 2 log reduction it
means that you are in CCR (Complete Cytogenic Response). If your result is a
3 log reduction it means that you are in MMR (Major Molecular Response).
Once you reach MMR there is virtually no chance that you will relapse with
your CML.

I assign Zero Club numbers when a patient achieves CCR.
