Both input and output are defined by the same means.

OK. I had assumed that a label reference at the start of a
specification only created an output stream. It appears from what you
say that a label reference always creates both input and output
streams.  Is this behavior documented?
Never mind - p 76 in Author's Edition. "The first reference to the label
creates the secondary input and output stream; the next one creates the
tertiary streams; and so on."
