Yes, this has been considered.

Can't do that since we'll have <pre> blocks with source code that will
dictate how big the width is. (I'm working with 80 columns <pre> blocks
just for testing but that's a problem with source-including HTML docs)

-1, underline is now considered part of visual semantics to indicate
hyperlinking and making a cursor change (or a hover effect) doesn't give
a global 'at-once' perception of the hyperlink space of the page.
