A short and light plastic stem, with a heavy bulb thing where you 
would normally find the point.  That has a flattened base so the pen 
stays upright when you let go, with an optical displacement sensor to 
detect how it is being moved.  The bulb part also has two tactile 
buttons, one for thumb and one for finger.     It would also have a 
tilt sensor so you can control scroll speed with it.  It would have 
to be wireless of course.  

Just a thought.  Personally, I don't have a problem with mice though!
