In Parser.g Line 1193, the ANTLR rules indicate that when identifier is not 
quoted, casing should not be kept:
`t=IDENT              { $name.setKeyspace($t.text, false);`
In order to resolve this issue, I will need to change this behavior and do the 
same for table-name.

Any objection?
