v4 renames convertEmptyIndexNamesToDefaultNames to addDefaultIndexNames and
calls it from all the index creation points before validateCfDef, including
QueryProcessor. No changes to the Migration classes are necessary.
Also fixes the deflate name creation to use the same index name convention,
which means it gets moved into CFMetaData (instead of ColumnDefinition) so it
has access to the comparator.
