Addressed the CodeQL findings in commit f3a2da9c77: widened index.numRows() before multiplication and used Arrays.toString(bindings.lhs) for the diagnostic message. The shared JUnit 4/JUnit 5 fixture lifecycle was also restored, and all 180 ChainedProcessorManagerTest cases pass.
